2017 Notices (if applicable)

• Safe Harbor 401(k) Plan
• Qualified Default Investment Alternative (QDIA)
• Automatic Enrollment

December 15, 2016:

Summary Annual Report (SAR) to Participants (if the 5500 was extended to 10/15)

December 31, 2016

• Plan Documents-Execute any 2016 Discretionary Amendments
• Required Minimum Distribution (RMD)
• New Plan Set-Up and Document Execution
